Our care pathways are designed to support you should you require long term support, as well as supporting you if you lead an almost independent life. Personalisation and choice are at the centre of our tailored approach empowering you to progress effectively toward greater self-advocacy, independence and autonomy.
How to make a Referral or organise a visit.
Making a call to us is the first step of the referral process. You will be able to discuss your client or family member in confidence with a professional who will become your key contact during the referral process. All clients receive a free pre-admission assessment which will establish their interests, needs, abilities and identify how we feel we could assist in providing a placement. We are happy to arrange for families and care professionals to visit us to meet our management and clinical team
We will work with the individual’s referring team and family to ensure a smooth and successful referral and transition to autonomy. We understand that not every referral can be planned and that some individuals may present in a state of crisis requiring urgent admission to a place of safety, we will aim to provide a rapid, accessible and responsive service in these circumstances.
Admission Criteria
We provide specialist services for adult males and females 18+ with a primary diagnosis of Learning Disability; Autism, including Asperger Syndrome; Mental Health Needs; or complex needs and behaviour that may challenge.
